HEALTH SERVICES

Dear Parent/Guardian,

I am looking forward to the 2013-2014 school year In order for me to best serve our new and current students, it is required that each student’s health and medication information be updated annually. To do this please fill out the Health Services Packet and return a hard copy to the school by August 1, 2013. If your child is to receive ANY medication while in school either non‐prescription or prescription, a signed medication form must be completed by their doctor.  If your child needs more than one medication please use a separate form for each medication - please do not put two medications on one form.  The health room will provide Ibuprofen and Acetaminophen as needed only with a signed doctor’s order/medication form. If your child requires an Epi-pen and/or inhaler, a medication form must be filled out and signed by a doctor, and the Epi-pen and/or inhaler must be provided to the health room for the school year.  Please be advised that under no circumstances will any medication be disppensed withoout a medication form signed by a physician as per Maryland State guidelines.
