- Required of all applicants to grade 9

All applicants to grade 9 take the Archdiocesan Admissions/Placement Test.
There is no pre-registration unless special accomodations are being requested Accomodation Request.

Transfer Application for Admission - For application to grade 10 or 11

Aquinas Program Supplement

Academic Recommendation - Print and submit to the applicant's current school (not applicable to students' attending schools of the Archdiocese of Baltimore)

Academic Recommendation for use by recommending school officials only

Request for Records - Applicants who are not currently students in a school of the Archdiocese of Baltimore must submit this form to their current school

Shadow Request/Registration Form - Complete and submit to request or confirm a "shadow" visit.
