• Spring Sports Tryout Information

  Prior to February 16th, ALL students interested in trying out for a spring sport must complete the Athlete Registration Form

  (Please note parents will not be able to access this form on behalf of the student)
  
  

  Additionally, any student who wishes to participate in athletics at Spalding must have the below information completed annually in their Magnus Health Account by February 16th.
  
  1. Vital Health Record

  2. Annual physical completed by parent & physician on Spalding Physical Form

  3. Medication Authorization Form (if applicable)

  4. Asthma, Allergy, Diabetes, Seizure plan (if applicable)

   

  All forms must be uploaded directly to the Magnus Health Account - paper or emailed copies will not be accepted. Magnus Uploading Instructions 

  
  Tryout dates, times, and locations for each spring sport will be posted below as that information becomes available.
